Where payment of premiums or levies is or will be made by credit card or pre-authorized payment, the following terms and conditions shall apply:

In accordance with the payment method and instalment plan chosen, the Payor authorizes Lawyers' Professional Indemnity Company (LAWPRO) to charge against the designated credit card or to withdraw from the designated account and financial institution, the insurance premiums (including applicable taxes) indicated. LAWPRO does not charge for this service (but the designated credit card company or financial institution may). This authorization form shall form part of the Law Society of Upper Canada Program insurance application(s) and POLICY/POLICIES of the Applicant and shall apply to the current POLICY PERIOD and shall continue to be used for and form part of, any and all future (annual or part thereof) renewal or replacement policies without further authorization by the Payor so long as the Applicant so elects. The Payor must read and understand the foregoing and the following terms and conditions:

1. The Payor certifies that the information provided in the authorization form is correct and that the Applicant (or Payor on behalf thereof) will notify LAWPRO in advance in writing (and no less than ten [10] days prior to any payment date) in the event of any changes.

2. The Payor certifies that the designated credit card or financial institution account is in good standing, with sufficient funds to cover the payments as they become due and that all person(s) required to provide authorization have done so.

3. All payments will be drawn on Canadian credit cards or financial institutions only, and will be withdrawn in Canadian funds.

4. The Payor and the Applicant acknowledge that LAWPRO may use and disclose their respective personal information referenced in this authorization form to each other (where Payor is not also the Applicant) and to the financial institutions involved in the payment process.
